  • Professional Development Plan
    • Describe your professional development plan in detail
    • Address how the proposed course will advance your workplace or professional goals
    • Include course/seminar information and the organization offering the education/training
  • Enrollment verification, class schedule and itemized tuition statement from the University/Organization providing courses. Please note that a generalized cost of attendance outline from the University’s website will not suffice.
  • A signed letter of support from the Principal/Supervisor
    • One-page maximum
    • Must be on official school/medical facility stationery
    • Confirms job description, status, and length of employment
    • Indicates support for the proposal and its impact on the applicant’s  development
